User's guide: Include Index in left pane

When I visit https://ghc.gitlab.haskell.org/ghc/doc/users_guide/, I see that I can get to the index by scrolling all the way down. But if I'm in a chapter, I can't do that, and scrolling all the way down is annoying. I think we should put a link to the index in the left side-bar.

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
