EPA: Remove AnnKeywordId

Alan Zimmerman requested to merge wip/az/epa-remove-annkeywordid into master

This was used as part of AddEpAnn, and is no longer needed.

Also remove all the haddock comments about which of are attached to the various parts of the AST. This is now clearly captured in the appropriate TTG extension points, and the ExactPrint.hs file.
