Document Olaf Chitils point about pattern matching against a polymoprhic argument

New compiler option -fignore-asserts: Causes

    (PrelGHC.assert pred expr)

to be rewritten to (expr).

Acknowledge the existence of both literal-literals and assertions.

- Document the pragmas we support
- Fix some lies in the vs. Haskell section.

Tweaks to make docs build cleanly

- add scoped type variables documentation.

Move 4.01 onto the main trunk.
