gitlab: Add issue template for backport tracking issues

As discussed with @osa1 and @RyanGlScott, I think our current handling of backports may be a bit fragile.

I propose we introduce the notion of backport tracking issues. Such a ticket will represent a set of patches to be merged to a stable branch.
