Add chapter on formation of committees + template

The next chapter from the Ways of Working Committee is on the process for the formation of Committees.

This text does not describe Task Forces, which would be created by the executive in completing a certain operational goal. By contrast, a Committee is a function of the Board to handle internal administrative matters of the HF.
