Minor refactor of CUSK handling

Previously, in getFamDeclInitialKind, we were figuring
out whether the enclosing class decl had a CUSK very
indirectly, via tcTyConIsPoly.  This patch just makes
the computation much more direct and easy to grok.

No change in behaviour.
